Department Description The UCSF Cardiac Rehabilitation Program provides medically supervised, multidisciplinary team based care that is customized for patients who have experienced heart attack, heart surgery, heart failure and angioplasty. This department will utilize the expertise of multiple professionals to address the following three goals in Cardiac Rehabilitative care: Exercise monitoring, counseling and training to support the continued cardiac health of the patient Nutritional counseling and support Behavioral counseling focusing on stress reduction. Job Summary The UCSF Cardiac Rehab Program provides medically supervised, multidisciplinary team based care that is customized for patients who have experienced heart attack, heart surgery, heart failure and angioplasty. Develops and implements an Exercise Physiology Program for the Cardiac Rehabilitation patients as part of the medical center's cardiac services. Provides information, education and training as necessary for patients, staff and the community. Functions as the team leader for the department, providing guidance, leadership and oversight to the Cardiac Rehab Department. Demonstrates expert clinical knowledge in Cardiac Rehab. Responsible for meeting program guidelines with the national Cardiac Rehabilitation organization and Medicare guidelines. Will provide clinical expertise for Cardiac Rehabilitation patients and the program. Exercises leadership for the interdisciplinary cardiac rehabilitation health care team and support services to deliver high quality, safe, effective and efficient family centered care. Provides cardiac rehabilitation services for the cardiac patients. Functions as a team leader to provide cardiac rehabilitation services to chronically ill heart patients and their families. The Cardiac Rehabilitation Nurse will coordinate the activities of the interdisciplinary team. Responsible for meeting program guidelines with the National Cardiac Rehabilitation Organization and Medicare guidelines. Interacts with leadership to assure guidelines are met. Required Qualifications Minimum of three years of recent clinical RN experience in the last five years Two to four years of recent Clinical Cardiology experience in the last five years One year experience in Phase II Cardiac Rehab Must demonstrate ability to address patient education needs of Cardiac Rehab patients Strong telemetry interpretation skills Active Registered Nurse licensure in the State of California Active American Heart Association CPR certification Bachelor's Degree in Nursing Demonstrated skills to effectively supervise clinicians and to manage the complex workflow and multiple priorities involved with a clinic(s) / center(s); strong knowledge of practice operations Demonstrate ability to address patient education needs of Cardiac Rehab patients In depth knowledge of patient care clinical standards and care delivery, systems, external regulations, and internal policies Strong knowledge of human resources management policies, with the ability to train, monitor, evaluate, and document human resources issues and performance, and to participate in decision-making on human resources matters Strong analytical and critical thinking skills, with the ability to quickly analyze problems, determine appropriate level of intervention, and develop and apply effective solutions Strong interpersonal skills for effective collaborations and problem solving with all levels of management, staff, consultants, and outside agencies Strong written and verbal communications skills, with the ability to train and mentor subordinates, and to convey complex clinical and technical information in a clear and concise manner Strong leadership, fiscal and operations management skills, with the ability to collaborate effectively with physicians, management and staff from all disciplines Ability to create and maintain a goal-oriented climate of teamwork, collaboration and consistent achievement of objectives Proven ability to organize, manage multiple priorities, and delegate work functions efficiently Strong telemetry interpretation skills The flexibility to orient and work at all UCSF Medical Center locations Preferred Qualifications Supervisory experience 1-2 years experience in Phase II Cardiac Rehab Active American Heart Association ACLS certification Knowledge of current and emerging best practices and technologies in the clinic / center healthcare industry Licensure/Certification Active Registered Nurse licensure in the State of California Active American Heart Association CPR certification Living Pride Standards Service Excellence Demonstrates service excellence by following the Everyday PRIDE Guide with the UCSF Medical Center standards and expectations for communication and behavior. Exhibits tact and professionalism in difficult situations according to PRIDE Values and Practices Demonstrates an understanding of and adheres to privacy, confidentiality, and security policies and procedures related to Protected Health Information (PHI) or other sensitive and personal information. Demonstrates an understanding of and adheres to safety and infection control policies and procedures. Assumes accountability for improving quality metrics associated with department/unit and meeting organizational/departmental targets. Work Environment Keeps working areas neat, orderly and clutter-free, including the hallways. Adheres to cleaning processes and puts things back where they belong. Removes and reports broken equipment and furniture. Picks up and disposes of any litter found throughout entire facility. Posts flyers and posters in designated areas only; does not post on walls, doors or windows. Knows where the Environment of Care Manual is kept in department; corrects or reports unsafe conditions to the appropriate departments. Protects the physical environment and equipment from damage and theft. The flexibility to orient and work at all UCSF Medical Center locations is required.
