The Care Management process emphasizes effective resource coordination across the continuum of care. The goals of this role include achieving optimal health, access to care, and appropriate utilization of resources. This must be balanced with the patients' self-determination while coordinating in a timely and integrated fashion. The role requires collaboration with patients, families, physicians, the interdisciplinary team, nursing management, quality teams, ancillary services, and third-party payers.
