Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 15 Shift: Day Hours per Shift: 10 Experience: 2 years of Case Management experience required License: Current RN Licensure in California Certifications: BLS certification required Must-Have: - Utilization Management experience - Experience working with attending physicians supporting level of care protocols Description: Responsible for Care Coordination and Care Transitions Planning throughout the acute care patient experience. This position works in collaboration with physicians, Utilization Managers, Medical Social Workers, and bedside RNs. The goal is to ensure the timely progression and transition of patients to the appropriate level of care to prevent unnecessary admissions or readmissions. The Care Management process emphasizes effective resource coordination across the continuum of care. The goals of this role include achieving optimal health, access to care, and appropriate utilization of resources. This must be balanced with the patients' self-determination while coordinating in a timely and integrated fashion. The role requires collaboration with patients, families, physicians, the interdisciplinary team, nursing management, quality teams, ancillary services, and third-party payers. This position is essential for addressing complex clinical and social situations efficiently to avoid unnecessary admissions.
