This assignment is ideal for an organized and compassionate RN who thrives in a fast-paced environment focused on care coordination and patient outcomes. Position Details: Start Date: 12/16/2025 Location: South San Francisco, CA Schedule: 5x8 Days Contract Length: 19 Weeks Key Responsibilities: Assess patient needs and develop individualized care plans to support safe and efficient transitions of care. Coordinate multidisciplinary services across inpatient, outpatient, and community settings to ensure continuity of care. Review clinical information, monitor patient progress, and collaborate with physicians, therapists, social workers, and insurance providers to remove barriers to treatment. Educate patients and families on treatment plans, follow-up care, and available resources to promote long-term health and stability. Unit Details: Care coordination role supporting patient-centered care delivery. Work closely with interdisciplinary teams to manage complex medical and social needs while ensuring appropriate utilization of healthcare resources.
