This role involves completing comprehensive Medicare Annual Health Assessments (AHAs) in both clinic settings and patient homes. You will use advanced diagnostic tools and Electronic Medical Records (EMR) to ensure thorough evaluations and collaborative care, directly enhancing patient outcomes without functioning as a Primary Care Provider (PCP). New Nurse Practitioner graduates are welcome to apply. (Student loan repayment assistance available for those who qualify) For San Francisco Bilingual Cantonese, Mandarin, or Vietnamese is required Why Join Our Team Company led by a practicing Nurse Practitioner (NP) who values the role and understands the challenges Mission-driven to address healthcare concerns including patient burnout, lack of quality care, and escalating costs Collaborative NP team across multiple states, partnering with Medical Provider Groups and Health Plans Less stressful work environment that supports high-quality education and patient care All necessary equipment provided, including laptops and tablets Additional benefits include malpractice insurance and cell phone reimbursement Job Duties Patient Care Work with geriatric patients Complete Medicare Annual Health Assessments at clinics and home visits Patient Load 7–8 patients/day in clinic 3–5 patients/day on home visit days This position includes seeing patients in their homes as well as in the clinic when needed Clinical Responsibilities Perform H&Ps Use preventive health screening tools (PAD and Neuropathy, diabetic retinal cameras, A1c and kidney testing for diabetics) Navigate and chart via EMR systems This position focuses on completing comprehensive health assessments and collaborating with patients' primary care providers to ensure continuity of care. You will enhance patient outcomes through detailed evaluations and effective communication, but this is not a primary care or PCP role.
