Job Description LanceSoft is seeking a local contract Physical Therapist for a local contract job in San Francisco, California. Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Duration: 13 weeks - 8 hours per week - Shift: 8 hours, days - Employment Type: Local Contract Essential Responsibilities - Conduct thorough inpatient rehabilitation evaluations, including functional assessments and mobility screenings. - Develop individualized treatment plans based on clinical findings and patient goals. - Provide therapeutic interventions to restore mobility, reduce pain, and enhance functional independence. - Complete IRF-PAI (Inpatient Rehabilitation Facility–Patient Assessment Instrument) documentation accurately and in compliance with regulatory standards. - Maintain timely and detailed documentation of treatment sessions, progress notes, and patient outcomes. - Collaborate effectively with physicians, nurses, case managers, and interdisciplinary rehabilitation team members. - Educate patients and families regarding rehabilitation goals, home exercises, safety, and discharge planning. - Adhere to hospital, departmental, and regulatory guidelines for patient care and safety. Required Qualifications - Current California Physical Therapist (PT) License. - Current American Heart Association (AHA) Basic Life Support (BLS) certification. - Doctor of Physical Therapy (DPT) degree or a Bachelor’s in Physical Therapy if graduated 15 years ago. - Minimum of 1 year of inpatient rehabilitation experience. - IRF-PAI experience required.
