In this role, you will provide personalized therapy services to patients in their homes, promoting mobility, independence, and quality of life. Your clinical expertise and commitment to patient care will make a direct impact every day. -Responsibilities:
  • Evaluate patients to determine therapy needs and develop individualized care plans
  • Implement targeted treatment programs to restore mobility, function, and reduce pain
  • Educate patients and families on treatment plans, home exercises, and preventative care
  • Monitor progress and adjust treatment as necessary to achieve optimal outcomes
  • Maintain accurate and timely documentation in EPIC EMR
  • Collaborate with physicians, nurses, and the interdisciplinary team to ensure coordinated care
  • Comply with regulatory requirements, departmental policies, and professional standards
  • Travel to patient homes; valid driver's license and automobile insurance requiredRequirements:
    • Graduate of an accredited physical therapy school
    • Current and active PT - Physical Therapist license in the practicing state
    • Basic Life Support (BLS) Certification - Healthcare Provider
    • 1+ year of Home Health experience required
    • Must have experience using EPIC EMR
    • Valid Driver's License (Class C) and current Automobile InsuranceSkills and Knowledge:
      • Critical thinking and complex problem-solving skills
      • Ability to work independently and manage a demanding caseload
      • Knowledge of professional practice standards and regulatory requirements
      • Strong organizational, verbal/written communication, and interpersonal skills
      • Ability to teach, develop training materials, and assess competencies
      • Proficiency with computers and required technologies
      • Ability to analyze data, develop treatment protocols, and implement evidence-based practices
      • Ability to collaborate effectively with a multidisciplinary team and stakeholders
