Job Description:
Employee administers appropriate nursing care residents in the skilled nursing facility under the supervision/direction of a registered nurse or physician. Performs a variety of direct and indirect patient care duties and activities. Supervises CNA staff.
Employees in this position require occasional supervision while working toward a definite objective requiring use of a wide range of procedures: plan, arrange, and/or determine specific procedures or equipment needs required to meet assigned objective: solve non-routine problems, referring only unusual matters to supervisor.
Education
1. High school graduate or equivalent.
2. Successful completion of an accredited LVN/LPN program.
Mastery of basic nursing skills. Minimum of one (1) year nursing experience and six (6) months acute or skilled nursing experience. Demonstrates good supervisory skills with CNA's, maintains good attendance record, seeks RN instruction when needed. Demonstrates good time management.

V. Major Duties and Responsibilities :
1. ccurately assesses patient's condition, and documents information obtained in the patient's chart. Administers medication, and provides patient care and treatments as ordered.
1.1 Performs vital signs and patient assessments accurately.
1.2 Properly prepares and administers medications correctly.
1.3 Demonstrates proficiency in administering skin and decubitus care.
1.4 Demonstrates proficiency in administering breathing treatments.
1.5 lways keeps attending physician informed of patient's condition,
reports any changes promptly.
1.6 ssists provider with exam and treatments as needed, and or directed.
1.7 cts effectively in emergency situations.
1.8 Proficiently and accurately performs specific laboratory and other
screening tests, treatments, and properly prepares specimens for
transportation to laboratory (blood glucose by finger, hemocult slides,
guaiac).
1.9 lways notifies ancillary services of physician orders or changes.
1.10 ccurately prepares requisitions for ordered tests, i.e. laboratory,
imaging, and respiratory.
1.11 Monitor fingernails on all patients, and cuts diabetic patients nails.
1.12 Provides complete reports on each patient to incoming shift.
1.13 Inventories and records all narcotics and medications.
1.14 ttends designated interdisciplinary team meetings.
1.15 Demonstrates understanding of state and federal regulations.
1.16 Provides input for MDS on assigned residents', on a regular basis.
1.17 Follows procedure in obtaining medications from Pharmacy.
1.18 Proficient in administering fluids by IV.
1.19 Reports laboratory and x-ray results to the resident's physician.
2. Proficiently, accurately, and timely documents patient care information in chart, and completes all required inventories, checklists, and reports.
2.1 ccurately completes monitoring of I& O's.
2.2 ccurately monitors vital signs, enter data and evaluates appropriately.
2.3 ccurately completes Behavior Sheets, and tallies.
2.4 ccurately completes Diabetic-glucose Record, and evaluates.
2.5 Reads and signs Daily Training Logs.
2.6 Documents communication with physicians, monitors and follows-up
s needed.
2.7 Completes Leave of Absence log, and charts for all patient leaves.
2.8 Completes required documentation in Nurses Notes, each shift.
2.9 Transcribes and notes physician orders appropriately.
2.10 ccurately completes Fall Risk Interventions and Post Fall Investigation
forms.
2.11 Documents medication and treatments given.
2.12 Documents effectiveness of "as needed" medication.
2.13 ccurately completes weekly summary reports.
2.14 Reads and signs Care Plans.
2.15 Writes and updates long and short term care plans.
2.16 Documentation is legible, neat, with few spelling errors.
2.17 Documents narcotic administration completely.
3. Proficiently performs other duties to assure skilled nursing unit runs at optimum operation.
3.1 Maintains appropriate and adequate supply of supplies, (with assurance that there is a minimum of one week's supplies, at all times).
3.2 Maintains adequate supply of medications.
3.3 Orders supplies accurately, timely, and in cost effective manner.
3.4 Properly cleans and stores, instruments, and if needed prepares
them for sterilization.
3.5 Maintains work areas, clerical and patient care, in orderly and
professional manner.
3.6 Removes date expired medication from stock and replaces.
4. Oversees Certified Nursing Aides staff.
4.1 Effectively coordinates CNA duties as needed.
4.2 ssures that CNA duties are completed before leaving for the day.
4.3 ssists, instructs, and advises CNA as needed.
5. Demonstrates effective delegation, coordination of workload.
5.1 Recognizes and deals with priorities promptly.
5.2 Demonstrates flexibility as workload and priorities change.
5.3 Executes work assignments in an organized, effective, and efficient
manner, demonstrating good time management.
5.4 Seeks help from co-workers when needed.
5.5 Completes tasks in a timely manner.
6. Complies with Infection Control policies.
6.1 Wears personal protective items as required.
6.2 Practices universal precautions at all times.
6.3 Checks items for sterility, outdates, and packaging defects.
6.4 Keeps adequate clean linen supply.
6.5 Safely disposes of contaminated items.
7. Complies with Quality Assurance and Confidentiality standards and policies.
7.1 ctively participates in SNF Quality Assurance program by informing
SNF supervisor of areas of concern, and offers suggestions for
improvement.
7.2 Demonstrates a high level of Confidentiality, and seeks direction when
presented with unusual circumstances.
8. Uses interpersonal skills effectively to build and maintain cooperative working relationships and enhance the District's image.
8.1 Works and communicates in a positive and cooperative manner
with management and supervisory staff, medical staff, co-workers,
other health care personnel, and patients and their families when
