Responsible for a caseload of participants who are under Pretrial Diversion and residing at the ECS Residential Re-Entry Program. Review and screen referrals to make recommendations to the Program Manager regarding admission. Perform intakes, assessments and develop Individual Service Plans for participants. Meet with participants on a weekly basis, providing and documenting individual and group services. Link participants to resources for income, primary care, substance use treatment, mental health treatment, legal, employment, clothing, housing, school, and any other resources participants need to successfully navigate the terms of their Pretrial Diversion and avoid reoffending and reincarceration. Ensure participants attend court dates and other appointments to ensure compliance with the terms of their Pretrial Diversion. Collaborate with Re-Entry Program Manager, Employment Specialist, Peer Specialists, Case Managers, Overnight Monitors and other team members to coordinate care.

PRIMARY DUTIES AND RESPONSIBILITIES:

Client engagement and support:

Treats participants with dignity and respect, communicating with them in a compassionate, non-judgmental manner.
Ensures participant conformance with program rules and regulations, department policies.
Monitor facility and respond to crises as required.

Clinical services:

Responsible for conducting assessments, creating and managing individual service plans, and counseling and crisis management.
Works with each participant to assess needs and develop/maintain service plans.
Meet with participants on a weekly basis or as needed to further treatment goals.
Facilitates and documents group and individual sessions with participants in accordance with service plan goals.
Coordinate with other non-clinical service staff for shared participants and to inform group intervention designs.
Coordinates transition plans and completes the discharge from the program.

Case management services:

Maintains a caseload of up to 20 participants.
Maintains client data and case notes in all relevant systems, and supports reporting of all relevant data to funders as needed
Identifies the need for other services and benefits (including literacy, computer competencies, ESL courses, high school diploma or equivalency education, or postsecondary education opportunities)
Makes referrals for other support services (including housing, legal, transportation, benefits counseling, childcare, substance abuse counseling)
Maintaining accurate records by entering documentation of all assessment, services, crisis management, and discharge services into various electronic systems for all guests in compliance with ECS practices.
Report participant progress to funders as required.
Other duties as assigned.

REQUIRED QUALIFICATIONS:

Previous experience providing full-scope Mental Health services billing Medi-Cal is strongly preferred.
Masters degree in social work or related field, with BBS registration (required) or licensure.
Experience working in integrated service teams using person-centered harm-reduction and trauma-informed approaches.
Strong background working with individuals who have been impacted by the criminal legal system.
Knowledge of Microsoft Office programs and experience documenting services into electronic record systems.
Operate with cultural humility in a diverse program and staff population.
Excellent working knowledge of houselessness resources in the Bay Area.
Possess a strong knowledge of and sensitivity to adults with substance abuse, domestic violence, incarceration, HIV/AIDS, mental health issues, history of trauma and co-occurring disorders.
Must have the ability to welcome and respect co-workers and client populations as unique individuals with different backgrounds, perspectives as well as differences in personalities, skill sets, life experiences, and knowledge bases.
Bilingual, Spanish/English preferred.
