Position Overview:
Under the supervision of the Clinical Manager, the Team Nurse is
responsible for the delivery of appropriate, adequate, effective,
efficient direct patient care services assigned by the Primary Care
Clinician and approved by the Clinical Manager. Responsibilities
include implementation of the physician plan of care, obtaining
physician orders, supervision of home health aides, and
coordination of care within an interdisciplinary team and
participating in effective utilization management and control of
patient supplies. Consistently provides care based on the
age-related needs of the unique population with consideration of
their development/psychosocial needs, environmental safety,
educational needs, communication style, and level of understanding.
The Team Nurse is accountable to the Primary Care Clinician for
ensuring that the plan of care is implemented following
professional standards of care and agency specific protocols.

EDUCATION
Associate's RN Graduate from accredited curriculum
Bachelor's Nursing or health related field preferred.
PREFFERED EXPERIENCE AS TYPICALLY ACQUIRED IN
Minimum of one year of experience in an acute care setting.
One year of experience in a Home Health or Hospice position
preferred.
Direct patient care experience within the past five years
required.
LICENSURES AND CERTIFICATIONS
Registered Nurse - State Licensure - RN
Driver License, Valid and in State - DL NUMBER
Automobile Insurance - INSURANCE
Basic Life Support Certification - BLS
Description
Exhibits excellent oral and written communication skills: with
legible handwriting for documentation of patient records in a clear
and concise manner.
Basic understanding of home care services, clinical standards of
care and practice, Title XXII, Medicare Conditions of
Participation, CHAP home health accreditation requirements, QI
outcomes/benchmarking, infection control; good understanding of
reimbursement patterns for Medicare and other payors; good working
understanding of budget and other financial reports.
Must have the ability to function independently and as a member of
an interdisciplinary team; ability to exercise independent, sound
nursing judgment in planning and providing patient care.
Exhibits knowledge of patho-physiology and accepted treatment
protocols for common HOME HEALTH diagnoses.
Knowledge of safety and infection control principles. Knowledge of
state and federal HOME HEALTH regulatory guidelines preferred.
Demonstrated clinical assessment skills.
Ability to recognize the special needs of Home Health patients and
others in the home.
Ability to communicate with patients and significant others in an
effective, mature, caring manner.
Ability to maintain harmonious constructive relationships with
internal and external customers.
Demonstrated effective written and verbal communication skills.
Understanding of palliative care for the terminally ill
patient.
Must be able to recognize unsafe or emergency situations and act
appropriately.
Participation in a training program for Home Health volunteers or
coursework in death and dying/Home Health care is preferred.
Must be able to handle sensitive issues, conflict with or among
others, respectively direct and reinforce staff efforts;
effectively plan and organize and prioritize work, think critically
to both effectively plan and organize department operations
consistent with Sutter VNA and Home Health's strategic and
operating objectives and to effectively solve unique problems as
they arise or identify when to consult supervisor.
Must be able to deal with challenging work environment with time
demands and occasional conflicting priorities.
Maintain a positive and professional image and demonstrate
self-directed learning to meet professional development and/or
license requirements.
Is responsible for maintaining all required licensure and
certifications.
At orientation and annually thereafter, must be able to meet Sutter
VNA & Home Health core competencies for position and area of
specialty, as appropriate.
Ability to be aware of surroundings and respond to emergency and/or
urgent events.
Ability to select and utilize patient transfer/transport equipment
safely.
Productivity standards - ability to meet productivity expectations
(3-6 pt. visits per day, depending on visit type and needs).
Must be detail-oriented.
Ability to assist with possibly disoriented, combative
patients.
Ability to work in mentally and physically demanding
environment.
Pay Range: 55.1-73.15
The salary range for this role may vary above or below the posted
range as determined by location. This range has not been adjusted
for any specific geographic differential applicable by area where
the position may be filled. Compensation takes into account several
factors including but not limited to a candidate's experience,
education, skills, licensure and certifications, department equity,
training and organizational needs. Base pay is just one piece of
the total rewards program offered by Sutter Health. Eligible roles
