Director of Community Engagement and Admissions, this position is
responsible for assisting the Intake Nurse in the referral process
which includes, providing functional guidance and direction to
Intake Assistants/Admissions Specialists; obtaining all the
pertinent demographic information; obtaining the payer source;
entering the referral in to Allscripts; assuring that referrals are
processed as quickly as possible; promoting optimal customer
service at all times. Essential Functions
- Assists with responding to incoming referral calls and
obtaining demographic information needed for the referral,
including patient and family member's demographics. Obtains
H&P, progress notes, D/C summaries and signed Certificate of
Terminal Illness. (CTI)
- Enters patient demographic information into Allscripts in a
timely manner
- Collaborates with the Intake RN to assure that referrals are
processed accurately and timely
- Fulfills all nursing responsibilities in absence of RN
- Performs quality control of staff maintenance of client files
and data input
- Provides education to community member call inquires about
Hospice philosophy, HEB services and Admission process
- Assembles and copies patient data timely. Maintenance of
organized records
- Maintains a system for reporting inquiries, referrals,
admissions and consults and monitor for accuracy
- Coordinates the distribution of tasks within the department and
monitors for accurate and timely completion. Structures work for
others around performance benchmarks and reporting deadlines
- Enters information for approved Intake/Admission staff monthly
schedules
- Runs weekly Admissions Referral /Conversion reports
- Monitors and updates Admissions and Bruns House boards for
accuracy
- Maintains system for tracking verbal CTI's to ensure that a
timely, returned, signed CTI has been received by HEB
- Ensures the CTI process is updated daily and monitors for
accuracy and timeliness.
- Maintains system for HEB clinical staff review of records
- Oversees and maintains admission charts and ensures they are
readily available for the admissions at all times
- Reads and responds to email and voice mail communications
timely
- Keeps Admissions Director updated as to the status of the work
being performed by the rest of the Intake Support Staff
- Supports Admission Director in the selection process of new
employees and orientating of new staff
- Leads Intake Specialists in team work activities regarding goal
setting
- Takes orders from PCPs, hospitalists, ECIN, consults, written
documents to "admit and evaluate if appropriate" and CTIs. Enters
in Allscripts
- Completes MSP (Medicare Secondary Payer) and initial
assessments under assessments tab in Allscripts
- Completes INTAKE note in Allscripts clinical notes verifying
which NP/MD patient/family chose to attend and follow
- Go out to consult with patient/family if no one else available.
Explain hospice and sign consents leaving the SOC (Start of Care)
date blank
- Go out and evaluate for Bruns admission if no one else
available. Complete Bruns admission process: call Bruns attending,
check with Bruns for availability and time of transfer, complete
consents and FAX all necessary paperwork to Bruns, inform intake
desk
- Print out and complete daily Query form
- General filing as needed
- Other duties as assignedPosition Requirements
- Must have the medical knowledge and critical thinking skills to
effectively assist with the process for referral to agency
services
- Understands and can communicate all agency services to caller,
have excellent interpersonal, communication and organizational
skills
- Detail oriented, able to prioritize, work independently and
follow through on tasks
- Able to handle sensitive issues
- A self-starter with a high degree of initiative, motivation and
flexibility
- Able to form harmonious, professional relationships with
internal and external customers
- Exhibit a "can-do" attitude toward assignments
- Knowledge and understanding of the hospice philosophy and
hospice criteria for admission
- Knowledge and understanding of the palliative care philosophy
and admission criteria for the program, knowledge of private
insurance, Medicare and Medi-Cal regulations as well as the Hospice
Benefit
- Able to remain sedentary at desk for 70% of day
- Must possess valid California license and reliable vehicle for
minimal travel
- Must possess appropriate automobile insurance
- Demonstrated ability to work all forms of clinical intake,
including electronic, telephonic and facsimile requests at
acceptable productivity and quality standards
- Must be able to work well under pressure, meeting multiple and
sometimes competing deadlines, demands and changing priorities
- Must demonstrate cooperative behavior with colleagues and
supervisors
- Must be flexible with work assignments and professional when
dealing with clients and staff. Ability to change direction quickly
as work needs change
- Must be able to coordinate and ensure that all work is
completed by end of day
- Must adhere to departmental goals, objectives and performance
standards
- Must have demonstrated leadership qualitiesExperience and
Education
- Relevant Associate's degree required; Bachelor's degree
preferred
- California LVN license required
- Minimum of one year relevant administrative experience in home
health or hospice agency, physician's office or hospital
required
- Knowledge of medical insurance companies and managed care
systems preferred
- Knowledge and experience with Microsoft Windows programs
required
