As we step into our new venture of home health, we are seeking an enthusiastic, empathetic, and driven professional Licensed Vocational Nurses (LVN) to provide intermittent services to patients at home following the plan of care.Ranked by FORTUNE as one of the Bay Area's 50 Best Places to work in aging services, our dedicated team is ready to grow!Responsibilities:

• Assist the Registered Nurse with total patient care
• Assist with the preparation, implementation, and continuing evaluation of the patient care plan
• Prepare clinical and progress notes
• Observe, record, and report patient symptoms
• Assist with the rehabilitation of patients according to the medical care plan
• Actively participate in nursing organizations, and promote and participate in in-service programs.Qualifications:
  • Graduated from an accredited School of Vocational Nursing.
  • Must be licensed in the State as a Licensed Vocational Nurse
  • Experience in a Home Health agency or hospital setting
  • Home health experience preferred
  • Must have current CPR certificationBenefits:

    • Available Shifts: Day Shift (7:00 a.m. to 3:30 p.m.) Evening Shift (3:00 p.m. to 11:30 p.m.) NOC (11:00 p.m. to 7:30 a.m.)
