Overall Summary LCSW or PsyD requiredPosition Available in Santa Clara County or Alameda CountyThis position is responsible for providing culturally competent behavioral health services to a diverse patient population, as well as clinical consultation to primary care patients with mental health needs.We are looking for a talented and highly-skilled clinician with excellent time-management skills and the ability to work with a wide variety of patients. Bilingual candidates are strongly encouraged to apply.Clinician will primarily be working with vulnerable populations and utilizing a brief intervention model, which focuses on providing 30 minute therapy visits. In addition, the clinician will be expected to perform warm handoffs as part of an integrated behavioral health approach to treatment. Training around the use of the brief intervention model (30 minute therapy sessions) and warm handoffs will be provided.This position is well-compensated ($100,000-$125,000 per year dependent on experience) and fits best with a clinician that is hard-working, thrives in a busy clinical environment, and is passionate about making a difference. Must be currently licensed as a professional LCSW or Psychologist (PsyD) within the state of California to apply.Essential Responsibilities* Provide therapy utilizing a brief intervention model (30 minute sessions) with patients.* Collaborate with other departments through the use of warm handoffs as part of an integrated behavioral health approach to treatment. * Consult and collaborate with primary care providers on the assessment and treatment of psychological disorders, substance use disorders, and psycho-social problems. * Consult and collaborate with primary care providers on treatment of chronic pain and chronic disease and the related psycho-social problems. * In consultation with psychiatrist, provide psychopharmacological consultation for primary care providers within scope of practice. * Provide consultation and training to clinic staff in all areas related to behavioral health. * Perform behavioral health screening, triage, evaluation, and assessment to determine appropriate level of behavioral health care. * Implement evidenced-based therapeutic methods, including cognitive behavior therapy, contingency management * Provide crisis intervention and behavioral health intervention. * Work closely with Medical Case Managers and providers. * Provide education and support to patients and their families to encourage adherence with medical and behavioral health issues. * Plan, coordinate, and facilitate referrals to existing community resources when indicated by assessed need. * Perform comprehensive behavioral health services including: intake evaluations, assessments, crisis intervention, ad hoc counseling, individual/family/group intervention, treatment planning, collateral assessment/ intervention, and outreach. * Develop and maintain procedures, protocols, and paperwork, forms and data on an on-going basis to ensure the program's relevance and usability to patients and clinic staff and funders. * Maintain current patient case records and prepare reports as indicated. Secondary Responsibilities* Reviews operations, procedures, interventions and documentation to adhere with agency, state and county guidelines and quality assurance standards. * Reviews clinical interventions and case documentation with Behavioral Health Director, participates in staff meetings, training, case conferences, and peer review process. * Participate in program evaluation, providing input and information as needed. * Attend workshops, training, and meetings as needed, and as requested. * Perform other related clinical and administrative duties as assigned by Behavioral Health leadership team. QUALIFICATIONS Required education, experience, training* Master's degree in social work/social welfare or doctoral degree in clinical or counseling psychology. * Current California license - clinical social worker (LCSW) or psychologist (PsyD) Preferred education, experience, training* One year of directly related experience and must be comfortable in a fast-paced primary care environment and working as part of an interdisciplinary team.* Experience in public/community health settings and/or in serving low-income multicultural populations * Bilingual candidates (preferably Spanish) strongly encouraged to apply. Physical DemandsThis position requires: * Sitting, walking, and standing associated with a normal medical/clinical office environment.* Regular bending and stretching; and lifting. * Manual dexterity used to include, but not limited to, standard office equipment, and computer keyboard. * Traveling to work sites including some exposure to outside elements.* Presenting information in front of a group. * Reading and writing in order to review records for accuracy.
