Job Description:

Our outpatient mental health community-based clinic located in downtown Oakland serving a largely Medi-cal population through a recovery-based, team model seeks a Psychiatric Mental Health Nurse Practitioner to join a team that includes a psychiatrist, nurse practitioners, clinical pharmacist, program nurse and case manager. This position will provide psychiatric services for our clients with moderate to severe mental health impairments. (1) Initial psychiatric evaluations of patients. (2) Formulate an initial diagnosis and treatment plan for individuals with common mental health problems. (3) Provide assessments, crisis response, medication and medication injections, direct mental health services, and medical record documentation, as required; (4) Formulates a corresponding progress note for each bill generated for any service that is provided. 5) Management and coordination of patient care, and work in conjunction with the Group Physician Lead or psychiatrist to develop the most appropriate medication regimen given the patients history and presenting symptoms; (6) Work closely with the patient and Group staff to help enhance medication adherence through patient education, one-to-one and/or group interventions with the patient and monitoring the medication procedures in place; (7) Assist in the implementation and maintenance of medication policies, procedures and protocols within the individual patient treatment plans as identified by the Medical Director or Physician Lead; (8) Monitor changes in patients with persistent mental illness, and notify the Physician Lead or assigned psychiatrist of any changes in a patients symptoms or impairment not managed by standard interventions; (9) Provide support, psychotherapy and supportive counseling for patients in regards to psychosocial, environmental, and physical well being; and Qualifications - Masters or doctorate degree from an accredited nursing school - Completion of an accredited Psychiatric Mental Health Nurse Practitioner program - At least 2 years of experience as a PMHNP, or will consider 1 year if was - 100% outpatient - Active California State License and DEA registration - NPI # and ability to obtain current Medicare and Medi-Cal number.
