Department Description The Division of Transplant consists of the liver, kidney, pancreas, small bowel, heart and lung transplant services. The Division's mission is to coordinate top quality care of patients in end stage organ disease and/or transplantation. The vision is to be the best provider and to expand the health care services to include the most diverse care available. The division currently has more than 100 medical center FTEs reporting to the Director. Job Summary The Nurse Coordinator leads the multidisciplinary team and is responsible for monitoring a subset of patients awaiting living donor kidney transplant. Provides expert nursing care for patients awaiting kidney transplant, including those involved in paired kidney exchange transplants. Serves as a contact point for patients, referring physicians, paired kidney exchange organizations, other transplant centers, and other staff. The coordinator facilitates the workup and care of the patient awaiting kidney transplant. Provides leadership for collaborative patient care, manages transplant logistics with emphasis on kidney donation and exchange. Provides education and continuity of patient care for the entire period of care including inpatient, outpatient or both. The coordinator shall evaluate the potential kidney donor or recipient telephonically and then proceed with viable candidates through tissue typing and Donor Work Up, Exchange, and Transplant. Required Qualifications Three years RN experience One year RN experience in the area of specialization, specifically outpatient organ donation or transplantation Active Registered Nurse licensure in the State of California Active American Heart Association CPR Certification Experience with organ donation or transplant logistics Experience leading staff education and training Demonstrated clinical competence including effective leadership, teaching, research, and communication skills The flexibility to orient and work at all UCSF Health locations Preferred Qualifications Experience in a large surgical practice Experience with process improvement strategies in healthcare Working knowledge of patient databases Apex and Teleresults Bilingual Licensure/Certification Active Registered Nurse licensure in the State of California Active American Heart Association CPR Certification Living Pride Standards Service Excellence Demonstrates service excellence by following the Everyday PRIDE Guide with the UCSF Medical Center standards and expectations for communication and behavior. Exhibits tact and professionalism in difficult situations according to PRIDE Values and Practices Demonstrates an understanding of and adheres to privacy, confidentiality, and security policies and procedures related to Protected Health Information (PHI) or other sensitive and personal information. Demonstrates an understanding of and adheres to safety and infection control policies and procedures. Assumes accountability for improving quality metrics associated with department/unit and meeting organizational/departmental targets. Work Environment Keeps working areas neat, orderly and clutter-free, including the hallways. Adheres to cleaning processes and puts things back where they belong. Removes and reports broken equipment and furniture. Picks up and disposes of any litter found throughout entire facility. Posts flyers and posters in designated areas only; does not post on walls, doors or windows. Knows where the Environment of Care Manual is kept in department; corrects or reports unsafe conditions to the appropriate departments. Protects the physical environment and equipment from damage and theft. The flexibility to orient and work at all UCSF Health locations is required.
