Job OpportunityTact Staff is seeking a skilled travel nurse RN
to fill a Case Manager position in Acute Care.Job Requirements:
- Specialty: Acute Care Case Management

- Shift: 12 hours
- Employment Type: TravelResponsibilities:The RN Case Manager
will be responsible for coordinating care, transitions, and
discharge planning throughout the acute care patient experience.
This includes completing timely assessments, updating notes, and
finalizing referrals to post-acute levels of care. The Case Manager
will also work with the multidisciplinary team to ensure a smooth
discharge process.Requirements:
- CA RN Licensure
- BLS (AHA)
- EPIC Experience
- 1 year of experience with knowledge of post-acute levelsWork
Schedule:The travel nurse RN will work 4 x 8 days, alternating
weekends, with variable shifts and no on-call requirements. There
may be opportunities to float to cover sick calls or PTO if census
