Clinic Director, you will be responsible for the oversight of
patient care in an assigned community or communities in accordance
with Federal, State, and Local regulations. The Clinic Director is
responsible for providing services (Therapy and/or Wellness) and
operational support to the rehabilitation service teams for
implementation of company-wide policies, clinical programs, and
quality initiatives to optimize each patients functional well-being
and satisfaction. As the Clinic Director, you may be responsible
for advising treatment planning and therapeutic interventions in an
interdisciplinary environment consistent with the positions
qualifications, professional practices, and ethical standards.
Essential Duties include the following: Delivers high-quality
rehabilitation and/or wellness services that are person-directed
and that produce outcomes in line with company goals and objectives
Develops and coordinates schedules of all services in community to
optimize resident participation and therapist productivity
Demonstrates solid knowledge and understanding across all
disciplines Acts as liaison between company and host community
through regular communication with Executive Director and Director
of Nursing/Wellness Promotes EmpowerMe service offerings and
provides in-services to care staff on a regular basis Obtains
consent and all necessary intake paperwork from resident or Power
of Attorney (POA) prior to initiation of services, including
communicating financial obligations as needed, and submits
documentation to business operations support team Provides summary
of residents currently on therapy caseload to Executive Director
and Director of Nursing/Wellness on a weekly basis Participates in
the assessments of new admissions, to identify any functional
issues that could be improved/addressed through therapy Actively
participates in all required meetings and trainings Attends daily
Stand-Up meetings and reporting information internally Participates
in the communitys regularly scheduled At-Risk Meeting to understand
what residents are at risk and may determine therapeutic
interventions that would be beneficial Participates in move-in
assessments with community as directed by ARD Maintains therapy gym
in tour ready condition Participates in quarterly reviews with ARD
Meets with families and patients as needed Uploads information from
daily Stand-Up form weekly If community does not hold Stand Up
meetings, check in daily with the team to find out the following:
tours scheduled where therapy involvement will be helpful,
assessments scheduled where therapy involvement will be helpful,
date of next At-Risk meeting Assists with meeting productivity
goals and expectations, as identified by the company Treats
patients and their families with respect and dignity; identifies
and addresses psychosocial, cultural, ethnic, and
religious/spiritual needs of patients and their families Complies
with HIPAA (Health Insurance Portability and Accountability) and
PHI (Personal Health Information) guidelines and protects
confidentiality at all times Provides input in formulating budget,
assists in evaluating department performance versus budget controls
and takes appropriate action to remain within budget guidelines
Communicates professionally, clearly, and maintains a good working
relationship with physicians, care staff, the administrative team,
and interdisciplinary teams, as well as demonstrates flexibility to
ensure patient/facility/customer needs are being met Identifies
residents who need therapy services through routine screenings,
communication with care staff, and monitoring of electronic medical
record(s) Implements clinical programs that are focused on the
needs of the residents living in senior living community
Administers and collects patient satisfaction surveys after
completion of therapy Communicates updates regularly to team;
monitors completion of required trainings Participates in
interviewing and onboarding of new team members, which includes
licensed therapists and assistants across all disciplines and
wellness coordinators. Participates in tours and helps prospective
residents and family members understand how EmpowerMe services will
address their specific needs Demonstrates competence in regulatory
compliance and provides assistance to Agency Administer(s), ARD,
and Clinical team(s) to aide in continued compliance with Rehab
Agency regulations within the Agency of participation Provides
leadership in such a manner that represents key servant leadership
principles, including effective listening, empathy, awareness, and
persuasiveness Delivers truthful and unfiltered information to
subordinates, supervisors, and company owners in a clear and
respectful manner Maintains professional expertise through
continued education as necessitated by licensure and as appropriate
for provision of specific treatments, utilizing EmpowerMes
educational resources when available Ensures that company standards
and policies are well-maintained, and aligns oneself with company
vision for service quality and value creation Performs other duties
as assigned Qualifications Degree in healthcare with at least 2
years of relevant past experience in a senior living setting
Applicable state licensure(s) Exceptional communication and
interpersonal skills are required along with the heart and passion
to work with older adults while providing the best care possible
Proven experience in staffing, leading, developing, and retaining
strong organizational teams Ability to work well with others and
take direction from management, as well as take initiative- willing
to go above and beyond to ensure the needs of the organization are
met in accordance with the companys core values Passion to achieve
results through self-motivation, initiative, and proactive
orientation with a sense of professional curiosity, desire to learn
new things, and to find/recommend solutions to problems Strong

offerings, Physical, Occupational, and Speech Therapy for seniors,
as well as a general knowledge of laws and restrictions regarding
Assisted Living, Independent Living, and Memory Care Must exhibit
and promote a high level of professionalism, customer service,
curiosity, and friendliness in all interactions with employees,
residents, and visitors throughout the organization Occupational
Therapist Qualifications, Education and/or Experience: To perform
this job successfully, an individual must be able to perform each
essential duty satisfactorily. The requirements listed below are
representative of the knowledge, skills, and abilities required.
Graduate of a bachelors, masters or doctorate degree in
Occupational Therapy with one (1) year of experience in
rehabilitation in senior care communities or related setting is
preferred Current state licensure, in good standing, as an
Occupational Therapist or be eligible for temporary license with
supervision based on current state guidelines Basic Life
Support/CPR certification Knowledge of Medicare payment systems and
EHR/EMR systems Proven ability to build and maintain a patient
caseload, specifically within a Med B setting Demonstrate
proficiency in occupational therapy modalities and ADLs Exceptional
communication and interpersonal skills along with the heart and
passion to work with older adults while providing the best care
possible Passion to achieve results through self-motivation,
initiative and proactive orientation with a sense of professional
curiosity, desire to learn new things and to find / recommend
solutions to problems Sense of professional curiosity, desire to
