Job Description:
Under the supervision of the Behavioral Health Manager or
delegated clinician, the Case Manager performs screenings and
limited case management tasks for patients in the Prenatal Clinic,
and limited case management task for patients/families in the Adult
Medicine or Pediatrics clinics. - The Case Manager will endeavor to
reduce the barriers patients encounter in meeting their needs
around Social Determinants of Health. - Essential Functions &
ResponsibilitiesThe Behavioral Health Case Manager has the
following responsibilities and duties:
- Assist MNHC patients/families to complete forms such as
disability benefits, food stamps, housing applications,
transportation assistance, emergency assistance, etc.
- Receives referrals/warm hand-offs from Behavioral Health
Clinicians and Primary Care Providers for concrete needs.
- Provide initial and follow-up Psychosocial Assessments which
include obtaining a psychosocial history and current situation and
a needs assessment for concrete services such as financial
benefits, housing, transportation, and other social services.
- Complete first-trimester psychosocial screening for patients in
the Prenatal Clinic and refer to relevant community services. - May
follow up with patients/agencies by phone or in person to address
barriers to access. -
- Timely and coordinated access to medically appropriate levels
of health and support services and continuity of care.
- Provide direct support services to clients through one-on-one
contacts-in-person, telephonically, warm hand off, referrals to
community resources.
- Assist clients in identifying barriers to services and help
them access needed services.
- Provide individual counseling and risk-reduction to clients and
significant other as appropriate.
- Provides crisis intervention and assesses clients for suicide,
danger to others and makes appropriate in-house or external
referrals.
- Ongoing assessment of the client's and other key family
members' needs and personal support systems.
- Empower/educate clients to make informed decisions regarding
health care and use of social services system.
- Accompany clients to appointments as needed and provide
language translation for clients when indicated.
- Work with diverse and underrepresented populations to
facilitate the access to medical and mental health services.
- Behavioral Case Management may also provide benefits counseling
by assisting eligible clients in obtaining access to other public
and private programs for which they may be eligible.
- Chart on all patients seen in the Electronic Health
Record.
- Attend multi-disciplinary team and departmental meetings and
other meetings as necessary or required by supervisor.
-Qualifications & RequirementsThe Behavioral Health Case Manager
should have the following skills, education and experience:
- Bachelor's degree in Social Work or related field (Master's
Degree preferred) -with one-year experience in case management
services.
- Bilingual (Spanish/English) required.
- Experience working with diverse populations, immigrants, asylum
seekers and the Latinx community preferred.
- Sensitivity to HIV/AIDS and Gay, Lesbian, Bisexual, and
Transgender issues.
- Ability to work professionally and ethically within
multi-cultural settings, including ability to maintain
confidentiality and privacy of persons, documents, data, and
