Responsible for obtaining and documenting theresident's
psychosocial history upon admission and updating quarterly and
asresident's status changes, arranging counseling services as
necessary. 2. Responsible for identifying and seeking ways
tosupport residents' individual needs and preferences, customary
routines,concerns and choices. 3. Responsible for arranging and
obtaining neededadaptive equipment, clothing, and personal items
including making referrals andobtaining services from outside
agencies. 4. Coordinates social services by participatingwith the
management team in care planning and development. This includes but
isnot limited to all MACC meetings. 5. Ensures that all entries in
the clinical recordare informative and descriptive of the services
provided, while maintainingorganized and orderly work records. 6.
Ensures comprehensive psychosocial and dischargecare plans are in
place for each resident within 72 hours of the completion ofthe
initial comprehensive assessment and updated as needed and
quarterly. 7. Assists in the preparation of the department'sbudget
and ensures that social services department operates within
establishedbudget. 8. Responsible for maintaining communication
andcontact with family, while encouraging their participation in
care planning. Education & Experienceyou need to qualify:
- A Bachelor's Degree in social work, humanservices or related
- Must be a full-time, qualified social workerwith a minimum of 1
year experience working in geriatrics in a facility withmore than
120 beds as well as 1 year of supervised social work experience in
ahealth care setting working directly with individuals.
- Excellent written and oral communication skillsare essential to
success. Ability to creatively problem solve and be a teamplayer,
well organized, and flexible.
- Able to communicate effectively, in English,with personnel at
all levels of the organization, with residents and theirfriends and
family, and with members of the community. Ways you can
advancebeyond the Social Services Director role: Once you master
this role, you could consider other moreclinical jobs needing
additional education such as Certified Nursing Assistant,Licensed
Practical Nurse, or Registered Nurse. There are other
